Helicobacter pylori (Hp) is the known bacteria living in the human stomach, and the infection rate in China has reached 50-67% (1,2). As the main pathogenic factor of chronic gastritis and peptic ulcer, it is also the Class I pathogenic factor of gastric cancer. Eradication of Hp has a preventive effect on gastric cancer (3,4). Moreover, it can effectively relieve the related symptoms caused by Hp in functional dyspepsia or other stomach diseases, and improve the degree of atrophic gastritis atrophy and intestinal metaplasia.
The eradication of H.pylori infection is based on the empirical antibiotic treatment strategy. The difficulties in the current eradication of Hp include an increasing resistance rate to many routine antibiotics, which leads to a decreasing success rate of Helicobacter pylori eradication year by year. The eradication rate of Hp in previous first-line triple therapy has decreased from more than 90% in the 1990s to less than 60% at present (3,4).
As an infectious disease, the best eradication for H.pylori infection is individualized treatment combined with the sensitivity of H. Pylori related antibiotics. Therefore, if the drug resistance information of antibiotics can be obtained before eradication treatment, no matter the initial treatment, initial treatment failure or the failure of eradication twice (refractory H. Pylori infection), doctors tend to recommend targeted drug selection based on drug sensitivity results, especially for refractory HP infection, family history of gastric cancer, or moderate and severe gastric mucosal atrophy, intestinal metaplasia or dysplasia found in gastroscopy. In this way, not only can satisfactory eradication efficacy be achieved, but also can effectively avoid unreasonable use of antibiotics and reduce the generation of antibiotic resistance.References:

As the chief physician of gastroenterology, the associate professor, postgraduate research supervisor, and associate director for Diagnostics Department of Shanghai Jiao Tong University School of Medicine, Dr. Xia has been involved in the work of clinical education and research over many years. She has previously been a visiting scholar at Johns Hopkins University, Mainz University and McMaster University. She is also the visiting associate professor of the Division of Gastroenterology at John Hopkins University.
Dr. Xia, is currently a member of the Chinese Society of Digestive Diseases of Chinese Medical Association (CMA), the Cooperative Group for Micro-ecology of the Chinese Society of Digestive Diseases of CMA, the Cooperative Group for Communication between Internal Medicine and Surgery, the longstanding Expert Committee for Digestive and Psychosomatic Disease of Beijing Medical Award Foundation, the Expert Committee for Health Management and Physical Examination of the Society of Endoscopy of Chinese Medical Doctor Association, the secretary-general of Inflammatory Bowel Disease of the Society of Digestive Diseases of China Association of Medical Equipment, the chairman of the education commitee of the China Crohn’s & Colitis Foundation, a member of the Expert Committee for Digestive Disease of Chinese Non-government Medical Institution, the first member of Shanghai Council of Chinese Digestive Psychosomatic Union, Dr. Xia has been a reviewer for professional journals including Chinese Journal of Digestion, Chinese Journal of Digestive Endoscopy, Chinese Journal of General Practitioners, and the Journal of Digestive Diseases. She is also the editorial board member for NEJM that is the Chinese edition of the New England Journal of Medicine, a member of WGe-WGN Editorial Board.

Dr. Chen is mainly engaged in the clinical diagnosis and treatment of reflux esophagitis, atrophic gastritis, functional gastrointestinal diseases, as well as liver, gallbladder, and pancreatic diseases; He is also experienced in the treatment of early gastrointestinal cancer and polyps under endoscopy. Besides that, Dr. Chen is proficient in endoscopic mucosal dissection and submucosal tumor resection, and has extensive experience in stent implantation and catheterization for advanced gastrointestinal tumors. In 2015 and 2016, he completed the endoscopic gastroesophageal myotomy (POEM) and the endoscopic pyloric myotomy (G-POEM). Dr. Chen’s annual endoscopic grade 4 surgery volume was over 200.
Dr. Chen is currently a member of the Youth Committee of Tumor Endoscopy of the China Anti-Cancer Association, a member of the Tumor Endoscopy Committee of the Shanxi Anti-Cancer Association, the Vice Chairman of the Youth Committee of Tumor Endoscopy of the Shanxi Anti-Cancer Association, and a member of the Endoscopy Branch of the Shanxi Medical Association.
