The Hepatobiliary and Pancreatic Surgery Service of Jiahui Health is committed to providing individualized comprehension diagnosis and treatment for patients with liver, biliary and pancreatic diseases. Our team comprises of professional and multidisciplinary members in this special field, including hepatobiliary surgeons, pancreas surgeon, gastroenterologists, radiologists, interventional radiologists and oncologists. We are dedicated to jointly developing treatment plans for patients and promoting their over-all health and well-being.
Dr. LI Jun, the speciality leading surgeon, certified by European Board of Surgery,, worked for over 20 years in the Department of Visceral Surgery at Essen University Hospital, Tübingen University Hospital, Hamburg University Hospital and Royal Free Hospital London. In 2018, the Section of Hepatobiliary Surgery at Hamburg University Hospital led by him became one of the top five liver surgery centers in Germany. Dr. LI Jun not only covers a wide range of diagnosis and treatment in general surgery, but also specializes in treatment of liver tumors, biliary tract tumors, pancreas and other abdominal tumors. Owing to years of transplantation career, his expertise of vascular reconstruction paves the way in challenging situation of advanced tumor and complex surgery. He also performes Minimally invasive surgery, including laparoscopy and Da Vinci robotic surgery regularily. Many of his innovative surgeries have been published and recognized in international professional journals.

We have an extensive cooperation and networkwith national and international experts and specialists in the filed of Hepatobiliary and Pancreatic Surgery , including top-tier 3A public hospitals with national key disciplines in Shanghai.
Jiahui Heath is fully equipped with advanced diagnostic technology, such as constrast ultrasound, Dual Source CT and 3.0 T MRI. Prior to the surgery, we routinely perform 3D imaging reconstruction for the liver and its surrounding tissues. A virtual surgery could provide all essential information on the important vessel, bile ducts and pancreatic duct. Observing from multiple perspectives and angles helps surgeons to preserve as much normal liver and pancreas tissue as possible. During the surgery, we use special probe for an intraoperative ultrasound exam, which means the ultrasonic probe could directly contacts the organ or the surface of the lesion, and detect the millimeter-sized tiny lesion that may not be detected by CT or MR. And the ICG fluorescent endoscope system is applied to localize the lesions and surrounding biliary tract, blood vessel.. To achieve a precise liver resection to millimeter level, we use Cavitron Ultrasonic Surgical Aspirator (CUSA) which is known in the surgery for brain. So we could selectively pulverize liver parenchyma while preserving the biliary tract and blood vessels in between. This precise operation helps to reach the removal of tumor tissue and the reduction of damage to surrounding tissue simultaneously. It significantly reduces bleeding and improves the success rate of clear and complete resection of tumor.

During the last 3 years, we have successfully performed many minimally invasive surgeries for liver, bililary tract and pancrease, as well as complex surgeries for requiring vascular reconstruction, including huge liver cancer, hilar cholangiocarcinoma and pancreatic head cancer. We work extensively with Medical Oncologists and Interventional Radiologists to provide individualized therapeutic plans for patients with advanced malignant tumor. The options consists of minimal invasive surgery, extensive complex surgery, palliative surgery, radiological intervention (such as ablation, TACE, HAIC)radiation therapy, chemotherapy, target therapy and immunotherapy, with respect to the quality of life of the patients.

Jiahui Hepatobiliary and Pancreatic Surgery Service covers common diseases and benign and malignant tumors of the liver, gallbladder, and pancreas, providing patients with various medical services of emergency, outpatient and inpatient.
The liver, gallbladder, and pancreas are digestive organs, and their symptoms of the lesions may include upper abdominal pain, abdominal distension, fatigue, jaundice, bleeding and wasting, and in severe cases, patients may have symptoms of ascites.

1. Liver
(1) Benign: Space-occupying lesions in liver (Hemangioma, Focal Nodular Hyperplasia, Adenoma), Hepatic Cyst, , Intrahepatic Stone, Fatty Liver, Alcoholic Liver Disease, Cirrhosis of Liver, Viral Hepatitis
(2) Malignant: Hepatocellular Carcinoma, Intrahepatic Cholangiocarcinoma, Liver Metastatic Tumor
2. Biliary tract
(1) Benign: Acute and Chronic Cholecystitis, Gallbladder Polyps, Gallbladder stones (cholecystolithiasis), Bile duct stones (Choledocholithiasis), Bile Duct Injuries
(2) Malignant: Carcinoma of Gallbladder, Bile duct cancers (Cholangiocarcinoma)

Jiahui Hepatobiliary Surgery Service provides outpatient, emergency, operation and inpatient care for patients over the age of 14.Medical Characteristics:
For patients with liver space-occupying lesions, gall stones or malignant tumors requiring surgical treatment, we offer various laparoscopic liver, biliary and pancreatic surgeries (like Cholecystectomy, Partial Hepatectomy, Pancreatic Neoplasms resection) as well as Complex Bile Duct surgeries and Liver and Pancreatic surgeries with vascular resection and reconstruction. In particular, wefocus on “initially unresectable tumors” by conversion therapy to regain the opportunity for surgery. Jiahui Hepatobiliary and Pancreatic Surgery Service use a comprehensive assessment approach to provide patients with the appropriate treatment for the best long-term outcome.
Examination/Diagnosis
●Imaging examination: Ultrasound examination is usually the first step to take. However, CT and Magnetic Resonance Imaging (MRI) , including Liver Specific MRI or Magnetic Resonance CholangioPancreatography (MRCP), offer an better overview and precise results. For take these exam, you need fasting for at least 4 hours, sometime is better overnight. Talk to your doctors for further information about getting ready for the exam.
●Needle biopsy: In some cases, the doctors need a sample of the tissue to confirm the diagnosis thus the proper treatment can go on. The easy way is a needle biopsy, which is carried out in local anesthesia with guidance of contrast-enhanced Ultrasound or CT. After the biopsy, certain observation time may be needed before you go home.
●Laboratory examination: Blood exam is a common method to help the doctors to know your medical condition, such as blood cell counts, liver function, tumor markers, virus load, cfDNA, etc.. The most reports w ould be issued on the same day within one working day with some exceptions (such as virus load, cfDNA).
●Genetic testing: Through the collection of tissue and/or peripheral blood, single gene or gene testing programs are available, such as HER2, KRAS, BRAF, NRAS, PIK3CA, KIT, PDGFRa, MSI, and Gastrointestinal and pan-cancer panel genetic testing programs. In addition, PD-1, PD-L1, Claudin18.2, TROP2, HER2, MMR and other Immunohistochemistry examinations.
Surgery
Preoperative: Using 3D reconstruction technology to restore the 3D image of organs, surgeons clarifies the anatomy, including the location and the size of lesion, and its relationship with surrounding tissues in advance, in order to better determine the scope of the surgery.
Intraoperative: Indocyanine Green (ICG) fluorescent and Intraoperative Ultrasonography (IOUS) are used for intraoperative localization, and surgeons use CUSA knife to accurately remove lesions while protecting healthy liver tissue in the most extend.

Dr. Li graduated at the Shanghai Medical University (now Fudan University), having the internship at Zhongshan Hospital and the Master degree at Cancer Hospital, Shanghai Medical University. He received the MD degree at the University of Essen in Germany and further the surgical training thereafter. He built his career in four prestigious European University Hospitals (Essen, London, Tübingen, Hamburg), with specialty of Surgery, General Surgery, Visceral Surgery, Hepato-Pancreatico-Biliary Surgery and Transplantation. Prior to Jiahui, he was in charge of Hepato-Pancreatico-Biliary Surgery at the University Medical Center Hamburg-Eppendorf, one of the five “Center of Excellence for Liver Surgery (DGAV)” in Germany.
Dr. Li has fundamental interesting in the surgical treatment of liver, biliary tract and pancreas disease. He made many surgical innovations. Some of them were published in the international prestigious journal. Applying the transplant technique and strategy on the treatment of cancers (as the part of transplant oncology) and performing minimal invasive surgery including robotic surgery (Da Vinci) are his current main focuses.
Dr. Li specializes in surgical techniques including but not limited to:
● Treatment of hepato-pancreatico-biliary malignancy, especially HCC, intrahepatic cholangiocarcinoma, perihilar cholangiocarcinom, gall bladder cancer, pancreas cancer and complex resection and reconstruction, including surgical palliation.
● Treatment of benign diseases of liver, biliary tract and pancreas, such as gall stone disease, iatrogenic biliary injury, intrahepatic stone disease, cystic liver lesion, portal hypertension. Many surgeries could be performed laparoscopically. Large part of liver benign tumor may not require a surgery. Some patients with jaundice can be treated non-surgically.
● Treatment of peritoneal and retroperitoneal tumor, which requiring reconstruction of large or small vessels, biliary tract, intestinal and even ureter.
● Treatment of gastro-intestinal tumor, including gastric cancer, colorectal cancer, tumor of the small bowel, gastrointestinal stromal tumor.
● Lymph nodes biopsy and lymphadnectomy in the area of neck, axillar, groin, abdomen or pelvis.
● Varies of laparoscopic operation, including liver resection, distal pancreas resection, gastrointestinal resection and reconstruction, cholecystectomy and hernia repair.

Based on his research, Dr. Pan published papers and gave presentations at a number of international academic conferences (including 2015 World Congress of Surgery (WCS) and 2016 Digestive Disease Week® (DDW)). Dr. Pan obtained his master’s degree in Surgery from Shanghai Jiao Tong University School of Medicine in 2005, during which he completed the internship and surgical training at Hopital Civil in Strasbourg. Prior to joining Jiahui Health, Dr. Pan was an attending surgeon in the Department of General Surgery at Shanghai Ruijin Hospital Luwan Branch.
